The Florence AyodeleAjao Foundation is seeking support to implement a 12-month Diabetes Care, Screening & Nutrition Support Initiative across underserved communities in Nigeria.

The programme will provide free diabetes screenings, health education, self-monitoring kits and affordable diabetes-friendly nutrition support to vulnerable individuals. Through community outreach and partnerships with healthcare professionals, the initiative aims to bring essential diabetes care closer to people who may otherwise struggle to access it.

• Direct Beneficiaries: 1,000 people
• Indirect Beneficiaries: 5,000+ community members

• 1,000 rural residents screened for diabetes
• 300 self-monitoring kits distributed
• 12 community education workshops
• 200 vulnerable individuals supported with diabetes-friendly food baskets
• Increased diabetes awareness within target communities.
